This patch contains fixes for the majority of remaining reported issues!

Changelog:
Fixed Door Ram not stunning nearby NPCs
Fixed players teleporting to the corner of the map when changing equipment while throwing
Fixed throwing delay and accompanying visual glitch
Fixed small, invisible holes in some walls that could be seen through
Fixed NPCs getting visually stuck in stunned state when stunned while stunned
Fixed NPCs occasionally being semi-visible when they should not be visible
